Specifications include, but are not limited to: St. Louis Public Schools is soliciting proposals from organizations that are qualified and interested inproviding:Case Management Case manager will collaboratively identify causes of targeted students difficulties, includingcircumstances unique to the student as well as those associated with school, family, or environment. Develop, document and implement a plan for supporting individual students. Problem solves issues related to housing and housing crisis, accessing community resources to assistwith basic needs as well as emergency situation, advocate for students and families in regards toreceiving proper health care, and assist in resolving matters of significance. Make referrals as needed for treatment, community agencies and other needed resources. Case managers will develop a plan of coordinated multiple services tailored to each student. This plangenerally includes a mix of short-term and long-range services that are delivered both in and out ofschool by the case manager and the community service network. Managers advocate for students and families by assisting and mediating student-family communicationswithin or outside the school. Advocacy also includes helping the student and/or family negotiate themany different bureaucracies. Managers will help to mediate and resolve conflicts and facilitatecommunication between students and family members, students and service providers The case manager will link targeted students and families to needed services which are not provided inthe school, drawing upon the community service network in arranging for services beyond the StudentSupport Team (SST) scope. Pre-referral counseling and family outreach activities help students and theirfamilies to accept services. In times of crisis, a team member accompanies the student and/or familymembers to the referral agency. The agency for the case manager will track services delivered to the student and family and monitor thestudents condition and emerging needs. As a result, adjustments in the service plan can be made andprogram milestones documented as circumstances dictate.
