Specifications include, but are not limited to: Shop Jacket $ $Shirt, Uniform Style, Short Sleeve $ $Trouser, Uniform Style $ $Coverall, Long Sleeve $ $4 x 6 Mat, Slip Resistant $ $12 x 15 Rag, Automotive Shop $ $15 x 18 Towel, Terry Cloth $ $36 Dust Mop, Collapsible w/o handleA. Upon initial award Contractor shall provide new, or like new, clothing. Uniforms supplied by theContractor shall not have stains, holes, tears, signs of excessive wear, or visible repair patches.B. Contractor shall meet with CHP Project Representative at the start of the Agreement to determine sizesand colors of items to be delivered. Contractor shall provide necessary staffing to measure each CHPemployee assigned the uniforms. Measurements will be taken at CHP, immediately after the award ofthis contract.C. Contractor shall deliver the initial order in full within one week (7 days) following the approval of theagreement with the exception of the uniforms. The uniforms must be delivered within 2 weeks (14days) following the employees fitting. The measuring of all uniforms for new employees shall beperformed within one week (7 days) of CHPs request. The CHP Project Representative will coordinateefforts with the Contractor.D. Contractor is not to offer any items not already identified in the Agreement. CHP shall not pay for anyitems ordered that are not identified in the Agreement. No other additional charges or fees will beauthorized for payment.E. CHP agrees to pay for the initial set-up of uniforms for each employee in the amount listed in Exhibit Bof this Agreement for this service. This initial set-up shall include: alteration of trousers, and employeename tag and CHP logo for shirts, coveralls, and jackets. Employee name tag shall be placed abovethe right, front shirt/jacket pocket (or where the pocket would be, if there are no pockets) and the CHPlogo is to be placed above the left, front shirt/jacket pocket (or where the pocket would be, if there areno pockets). The set-up of replacement uniforms shall be at no additional charge to CHP.F. The CHP Project Representative shall provide Contractor all required information to be placed onuniform emblems and name tags.
