Specifications include, but are not limited to: 2.1.1The contractor shall provide transitional living program services appropriate for older youth (hereinafter referred to as youth) who are in need of guidance, coaching, and mentoring in a safe and supervised living environment to learn independent living skills in a community setting for the Department of Social Services, Childrens Division (hereinafter referred to as the state agency). a.The contractor shall serve youth who present a variety of behaviors, which may include, but not be limited to: poor impulse control, academic under achievement, dysfunctional family relationships, conflict with authority, sexually promiscuous, conflict with peers, lack of attachment, passive and/or aggressive behaviors, substance experimentation, disrespect for property, weak boundaries, poor self-image, poor work ethic, mental health issues, non-realistic about goals, limited problem-solving skills, and parenting teens. b.If requested by the Foster Care Case Management provider, the contractor shall provide transitional living program services to the youth case managed by the state agencys Foster Care Case Management agencies.2.1.2The contractor shall perform the awarded transitional living program service(s) in the awarded region(s) on an as needed if needed, basis in accordance with the provisions and requirements stated herein. The contractor shall perform all services to the sole satisfaction of the state agency. a.In the event multiple contracts exist per region, the decision of which contract to be utilized shall be determined as described in the Referrals Requirements herein.2.1.3The contractor shall provide one or more of the following transitional living program service(s), as specified in the Notice of Award issued by the Division of Purchasing (hereinafter referred to as Notice of Award). The contractors transitional living program services must satisfy the following definitions: a.Transitional Living Group Home - A licensed and contracted Residential Child Care Agency operating a site dedicated solely to youth, ages sixteen (16) through age eighteen (18) until the nineteenth (19th) birthday, who have a case plan of independent living. The contractors program shall allow the youth to learn and practice life skills in a group setting with on-site supervision in accordance with residential child care agency licensing rules 13 CSR 40-71.010 through 13 CSR 35.71 which may be found on the internet at The number of youth served at a given operating site shall not exceed the licensed capacity of the site. The contractor shall not place more than two (2) youth per bedroom unless allowed by licensure; however, it is preferred there be only one (1) youth per bedroom. b.Transitional Living Single/Scattered Site Apartments (hereinafter referred to as apartments) - An approved and contracted living arrangement dedicated solely to a youth(s), ages eighteen (18) through age twenty (20) until the twenty-first (21st) birthday, who has a case plan of independent living and has demonstrated an ability to live independently with an array of supportive services. The contractors program shall allow the youth(s) to learn and practice life skills autonomous from the group setting with oversight, but not necessarily on-site supervision. The contractor shall not place more than one (1) youth per bedroom occupying a given living arrangement unless otherwise approved by the family support team, the youth, the state agency case manager, the contractor, and the state agency. 1)For purposes of this document, a scattered site apartment is defined as a self-contained furnished (refrigerator, stove, bed, oven, table, chairs, etc.) unit with utilities (trash, sewer/water, electricity/gas, phone accessibility, etc) cooking, sleeping, and bath facilities for no more than one (1) youth per bedroom. Scattered site apartments may include apartments adjacent to other treatment programs, or sites scattered throughout the community and/or apartments clustered together.2.1.4If the contractor provides services other than transitional living programs (e.g. Residential Treatment, Day Care, Foster Care, Crisis Care, etc.), the contractor must provide the transitional living program services separately from the other services and may include off campus programming and/or additional operating sites.2.1.5The contractor shall work in collaboration with the youths case manager to provide the services required herein. The contractors responsibilities shall not replace the responsibilities of the youths case manager.
