Specifications include, but are not limited to: The City of Ogden looks to install new sports field turf at Liberty Park. Liberty Park is one of the oldest parks in the city inventory and it is being repurposed to allow for new recreation opportunities. The installation of new features at the park will ensure that the park will continue to be used for generations to come. Some of the new functions are: new restroom, new basketball court, two new lacrosse / multipurpose fields and play areas. In addition a new walking path will be installed along the perimeter. The purpose of this RFP is to complete the Sports Field Turf installation of this project. PROPOSAL CONTENT: Proposals shall contain the following: A. A letter of transmittal including the Service Providers name, address, telephone number, fax number, e-mail address, business license and professional registration numbers, and a statement that the proposal will remain in effect through ninety (90) days from submission deadline until a contract is executed, or until the procurement is terminated, whichever comes first. B. Name, association with the individual or firm and work experience of all key personnel who would provide service to the City under this contract. C. Acknowledgment of receipt of addenda (if any). D. Listing of similar projects performed by the Service Provider. E. Investigations, arbitrations, litigations, or claims, which are pending, settled or otherwise, disposed of within the last three years.F. Proposals shall be submitted under the same cover at the same time, in the following distinct sections: (A) Product: Shall include product catalogs and or cut sheets of proposed materials to be used, confirmation of the SCOPE OF WORK process located in this document, warranties, and submittal forms for any product that deviates from the basis of design. (B) References: Five (5) References for completed similar size and scope of this project and that the vendor (local Company, not national manufacturer) has supplied and installed like materials for within the last 48 months. These references shall include site name, contact name, and contact information, including e-mail address and phone number. (C) Cost: The cost shall be broken down into product cost, installation and possible discounts. Proposals are to be prepared on standard 8-1/2 X 11 paper. Fold-outs containing charts, spreadsheets, and oversize exhibits are permissible. The pages should be placed in a binder with tabs separating the sections of the proposal. All responses, as well as any reference materials presented must be written in English. Total of 3 copies. H. Proposals shall be based only on the material contained in this RFP. I. Responses to this Request for Proposal shall be submitted to the office of the City Purchasing Agent, 2549 Washington Boulevard, Suite 510 (5th Floor) Ogden, Utah, no laterthan 2:00 p.m. onFriday March 25th 2016. LATE PROPOSALS WILL NOT BE ACCEPTED. J. Proposal Evaluation will be based upon the following: Responsiveness of design and product selection based on the criteria herein Materials proposed Schedule proposed by candidate Past performance : Experience, key personnel, and references Cost: Overall cost in comparison to value K. Deliverables to be accomplished in 2-4 weeks or less from NTP.
