Specifications include, but are not limited to:Customize the NCSEAM Early Intervention Family Survey, (e.g., revising severalsurvey items or terminology to meet State data needs) and the Family Survey envelope (e.g., MSDE andthe Maryland Infants and Toddlers Program on the envelope), as requested by MSDE. MSDE will usethe Impact of Early Intervention on Your Family Scale (22 questions), plus several additional surveyitems. These two additional items will be prefaced by the following statement: Only complete thefollowing questions if your child turned 3 years old before July 1, 2015 and your family continues toreceive early intervention services through an Individualized Family Service Plan (IFSP). Theadditional survey items will address two additional family outcomes for families who chose to continueto receive early intervention services after their child turned 3. The additional questions will include:Over the past year, Early Intervention services have helped me and/or my family: a) Understand myoptions in order to make the best choice for my child and family to continue services through anExtended IFSP or move to services through an IEP; and b) Support my child to be ready for school byassisting me to teach my child pre-reading (such as naming pictures) and pre-math activities (such assorting household items) during daily routines. Work with MSDE, DSE/EIS, Contract Monitor to draft acustomized cover letter (English/Spanish), including local and State contact information, to be given tofamilies along with the NCSEAM Family Survey. In addition to the development of a paper survey, develop an on-line, Web-basedsurvey that shall include3. Basic Web survey development Database development Additional items incorporated i.e. MSDE logo, pictures, etc. Real-time response progress monitor Spanish version (text) Host survey (Set up login accounts) Technical support Security (Secure Socket Layer with 128-bit encryption) Features to ensure non-duplicative completion of the survey
