Specifications include, but are not limited to:The State mandated Intoxicated Driver Resource Center (IDRC) Affiliation Agreement serves as the application. Therefore, to apply to become a Somerset County Affiliated IDRC provider for outpatient and/or intensive outpatient substance abuse service, please complete the checklist and the agreement application as attached to this document. Please be advised that there is no Somerset County funding attached to this affiliation process. If you are approved to be an affiliated provider, you will be listed on the affiliated provider list. This affiliated provider list will be presented to all applicants who meet the criteria for further evaluation as determined by the State. Notice of approval will be provided once signed applications are returned from the New Jersey Department of Human Services (DHS), Division of Mental Health and Addiction Services (NJDMHAS), Intoxicated Drivers Program (IDP). It is anticipated that the approvals will be made during the month of June 2016. The Affiliation Agreement will be in effect for one-year, July 1, 2016 to June 30, 2017 for non-DHS licensed providers. DHS licensed substance use disorder ambulatory treatment providers may be renewed annually for a period of up to five years annually if all other conditions of the contract are met; those providers that are eligible will be alerted. The following criteria are applied to be considered a Somerset County IDRC affiliated provider: 1. The Somerset County Department of Human Services (SCDHS) and the Board of Chosen Freeholders must accept the Affiliation Agreement. 2. The agency/provider seeking to become a SCDHS IDRC affiliated provider must have a site within the Somerset County geographical boundaries at which services are provided to IDRC clients. 3. The agency/provider must be a New Jersey Department of Consumer Affairs licensed provider or a New Jersey Department of Human Services (DHS), Division of Mental Health and Addiction Services (NJDMHAS) licensed agency. 4. Each affiliate agrees to monitoring by the Somerset County Department of Human Services, and/ or State, at least annually and more if required i.e., concerns, complaints. 5. The agency/provider must provide a minimum of 10% free/low fee services to IDRC clients.
