The purpose for this RFP is to select a contractor to develop a network of Regional Health Connectors (Connectors) that will serve as brokers of practice transformation health extension activities that align with the states Regional Service Area (RSA) boundaries.The Connector network will advance regional care delivery redesign priorities by assessing needs, providing technical assistance, and coordinating an efficient referral strategy to coaching, facilitation and training services that will address practices learning priorities. The Connectors will serve as drivers of change, promoting the utilization of the Practice Transformation Support Hubs (Hub) practice transformation portfolio of curated training, proven tools, and robust practice coaching and facilitation services.. The Contractor will perform work as outlined in the Scope of Work, incorporated herein as Attachment A. This scope of work represents the anticipated work to be done for years one, two, and three. Bidders are encouraged to draft responses that show how the objectives of the SOW will be met with creativity, innovation and efficiency, including leveraging existing best practices and resources.No additional specifications were given.
