Specifications include, but are not limited to:Applicant: SAMPLE APPLICANT Department: Department of State Hospitals Job Classification: InvestigatorPsychological Evaluation and/or Treatment History: On the PHQ, #4, the applicant indicated that she had been to counseling. Treatment records were sought and received from Dr. SB, EP, MFT, and Dr. L. The AP was reportedly in individual therapy for about three years when she was a teen related to her parents divorce. Due to remoteness, no records were available to review. Subsequent treatment notes reference that the AP was estranged from her father and that she witnessed domestic violence. A close friend of the APs collapsed and died while they were dining together in 2009. She sought bereavement counseling through her EAP with Dr. SB. They met three times. When her EAP benefit was exhausted, she sought treatment at Kaiser with EP, MFT, whom she saw for five visits between July 2010 and June 2011. She was diagnosed as Bereavement, Uncomplicated. She reported to Ms. EP that she suffered flashbacks and nightmares for about a month after her friends death. At the time of this therapy she was no longer disturbed by either dreams or flashbacks but felt bereft. She also expressed worry that she, too, may suddenly die. In August 2015, the AP sought counseling services with JL, PsyD, for stress she was feeling in her job working in internal affairs for DSH. Dr. L. diagnosed her as Adjustment Disorder with Anxiety. It was also documented that the AP was feeling stressed by the rigorous psychological interviewing and testing she was doing in preparation for the police academy.
