Specifications include, but are not limited to:Prior to the contractor performing services, the contractor and state agency shall meet at the mutually agreed upon service facility and go over the examination requirements.The contractor shall administer the pre-employment fit for duty examinations as follows:Consent to Examination Upon arrival of the individual requiring a pre-employment fit for duty examination at the service facility, the contractor shall have the individual sign a consent form.Medical History The contractor shall perform a medical history questionnaire on the individual.If anything in the individuals health history is cause for concern by the contractor, the individual must get a physicians release form prior to performing the examination. If the contractor has a physician on site and the individual wishes to use the contractors physician, the services must be coordinated between the individual and the contractor. The individual shall be expected to pay for the cost of the physicians exam. Neither the contractor nor the state agency shall be responsible for the individuals physician costs related to getting the physicians release form. The individual will be solely responsible for this cost. The contractor must let the state agency know the examination is on hold while the individual obtains a physician release form.Heart Rate and Blood Pressure The contractor shall take a resting heart rate and blood pressure of the individual.If the individual has an abnormal heart rate and blood pressure, the contractor shall end the examination. The contractor shall not continue the examination until the individual has a physicians consent to continue with the examination. An abnormal heart rate and blood pressure shall be as indicated in the contractors awarded proposal.Height and Weight - The contractor shall record information on the individuals height and weight.Grip Strength The contractor shall administer a grip strength test. The grip strength test shall be for baseline information.Musculoskeletal Screen - If an individual reports during the medical history interview/questionnaire a previous or existing injury that may be limit the injured body part, the contractor shall understand and agree that a licensed medical provider such as a physical or occupational therapist shall give a musculoskeletal screen to ensure the individual is safe to complete the examination and document a baseline status.Box Lifts - The contractor shall go over with the individual proper lifting, then the individual shall be asked to lift weighted boxes from floor to knuckle and knuckle to shoulder. The contractor shall give the individual lighter weights to start with and then shall progressively add weight until the individual can lift 70 pounds floor to knuckle and 40 pounds knuckle to shoulder. The individual does not need to lift the final weight of 70 lbs and 40lbs more than once.The contractor shall conduct the following five (5) separate tests to complete the box lifts.20lbs vertical lift from floor to knuckle height45lbs vertical lift from floor to knuckle height70lbs vertical lift from floor to knuckle height20lbs vertical lift from knuckle to shoulder height40lbs vertical lift from knuckle to shoulder height
