Specifications

Specifications include, but are not limited to:The Consultant shall be licensed by the appropriate State Board to practice podiatry in the Commonwealth of Virginia and certified by an appropriate specialty or sub-specialty National Board. The Consultant will provide current copies of such licenses, certifications, registrations, malpractice insurance certificate, and accreditation (if applicable) to Western State Hospital.B.Consultant to provide services two (2) Wednesdays per month between the hours of 8:30a.m. and 12:00p.m. These hours are adjustable based upon need, number of patients scheduled, etc. Variations of this schedule shall be mutually agreed upon between the Contractor and the Supervisor of the Medical Consultation Department.C.Consultant shall provide written or dictated reports for consultation.D.Consultant shall provide a copy of his current licensure as a Doctor of Podiatry in the State of Virginia and those of associate podiatrists practicing at the Agency.E.Per the requirements of The Joint Commission for contracted services, the Consultant shall provide upon request, health status information as follows:1.Annual Tuberculosis screening (TST) result-WSH will administer if desired.2.Hepatitis B vaccination status and titer or signed declination-WSH will administer if desired.3.Date of last tetanus immunization-WSH will administer if desired.4.Varicella immunization or history of disease.5.Annual influenza immunization is preferred and strongly encouraged-WSH will administer if desired.F.Consultant shall provide verification of liability coverage.G.Consultant shall agree to practice within the guidelines of the by-laws, rules and regulations established by the Medical staff of WSH.H.Consultant may be required to confer clinical privileges to practice at WSH, which may include;1.Authorization for representatives of the Agency to consult with others associated or familiar with the Contractors qualifications.2.Authorization for representatives of the Agency to examine credentials at other hospitals where the Contractor has privileges to practice.I.Consultant shall obtain approval of the Director of Primary Care Services to perform podiatric procedures that would be considered other than routine office procedures.J.Consultant shall agree to and sign Attachment D Associate Business Agreement.K.Agency agrees to Enable Contractor to practice podiatry on the ground of the Agency.
