Increase employee and dependent voluntary participation in primary and preventive care/screeningservicesD. Effectively integrate a clinic with currently offered health care plan elements.E. Provide employees the tools and information to make educated decisions on their healthF. Reduce Daytons cost in the long term while enhancing healthcare options now and in the futureG. Provide an enhanced method of healthcare delivery by establishing an On-site or Near-site healthand wellness clinic for Dayton employees and their eligible dependents who volunteer toparticipate.Dayton desires a health center which provides a care and payment model that includes 45/5 ease of access to boardcertified family practitioner healthcare providers, while offering same day and next day appointments in exchangefor a per member per month fee (without submittal of claims). Dayton offers a high deductible health plan (HDHP)with a Health Savings Account (HSA) so offering these services while still keeping the employees HSA compliant isvery important. The organization selected is expected to provide feedback on how other companies have offered anOn-site or Near-site clinic alongside a HDHP, and explain how they have handled similar arrangements at otherclinics. In addition, the clinic should offer on-site, low cost, generic medicine management and basic laboratoryservices. The organization selected should make online health tools (i.e. patient portal) available to members.Overall the model selected should result in significant healthcare savings for Dayton if well utilized. Dayton intendsto follow a selection timetable that may result in the choice of an on-site or near-site clinic vendor this fall, with thevendor expected to have a full clinic operational by June 1. Finalists may be invited to present a more tailoredproposal to Daytons administration, including representatives from management and labor representatives and theHealthcare and Wellness Committee.
