Specifications include, but are not limited to:The Contractor shall provide Occupational Therapy Services, an estimated 20 hours per week, on the premises of the SoutheastArkansas Human Development Center (SEAHDC) in Warren, Arkansas.The Contractor shall comply with all State and Federal ICF/MR Standards for Occupational services.The Contractor shall comply with all statues, regulations, codes, ordinances and licensure or certification requirements applicable to theContractor or to the Contractors agents and employees and to the subject matter of the contract. Failure to comply shall be deemedunacceptable performance.Therapist must participate in the Individual Program Plan (IPP) Development which entails the initial program planning (30 day review)for all individuals residing at the facility and all new admission.Therapist must attend Interdisciplinary Team meeting as requested by the Program Coordinator. Therapist shall work with the facilitysinterdisciplinary team members to develop occupational training goals and objectives. Treatment plans are to be developed with eachindividuals primary physicians approval.Therapist shall attend medical staff meetings and conferences as required.Therapist will complete evaluations on each individual determined to need services. Evaluations are to be done on individuals within thirty(30) days of admission and every three (3) years on individuals under the age of 21 and every five (5) years for individuals twenty-one (21) andolder. Therapist shall submit all required annual evaluations a minimum of seven (7) days prior to the individuals annual review.Evaluations shall include written training plans and proposed objectives in terms of developmental programming and supports to enablethe individual to adapt to his/her environment.Therapist will report at a minimum, monthly, the individuals progress on any/all stated therapy objectives.Therapist is to provide emergency (on-call) services for individuals at SEAHDC when an emergency arises.Therapist will provide all necessary in-service training and orientation as determined by the Interdisciplinary team to facilitate progresson established occupational therapy objectives.
