Specifications include, but are not limited to:3.1 Access, Membership Criteria and Service EnrollmentThe Brain Injury Community Center Membership is open to adults who can document an acquired brain injury, are interested in participation, and are not a risk to others in any way.Individuals who have a traumatic externally caused head injury will be referred by MRC, after eligibility has been established through the Statewide Head Injury Program (SHIP). The Contractor must inform individuals that, if they accept the Community program, information about them and the services provided will be submitted to MRC, since MRC is the sole or primary funding source for the Community program.The Contractor can receive referrals from anyone whose acquired brain injury is not traumatic, and the individual can request services on their own. The provider is responsible for verifying the occurrence of a brain injury. The documentation is retained in the Members records.If at any time the Contractor determines that an individual poses a significant and current threat to the general safety of the Community Center, the Contractor shall consult with MRC Coordinator as to appropriate actions to be taken.Priority for membership will be given first to those on MRCs current waiting list. The Contractor will collaborate with MRC regarding service enrollment. As part of this pilot, the role of MRC staff in relation to Members who do not have traumatic brain injury will need to be developed.3.2Member EnrollmentMRC Referred IndividualsMRC will refer an individual to the Contractor using a Brain Injury Community Center Referral Form. MRC will also forward all relevant demographic and clinical information it may have available. The individual shall be deemed as meeting the Membership Criteria, and the program Referral Form shall be used by the Contractor as verification and documentation of such.Referrals from Other SourcesIf an individual is not referred by MRC, the Contractor must verify that the individual meets the Brain Injury Community Center Membership Criteria set forth and completion of his/her enrollment must occur within thirty (30) business days of the individual being referred to, or requesting, the program. Verification of a brain injury by a clinical record is required and must be attached to the Referral form, which is sent to MRC. The processes for determining that an individual meets the Brain Injury Community Center Membership Criteria and completing his/her enrollment must be tracked by the Contractor.For an individual to become a Member, the Contractor must complete and submit to MRC for approval a Referral Form. Within ten (10) business days of the receipt of the form, MRC will make a determination, and will sign the Brain Injury Community Center Referral Form noting the determination and return it to the Contractor.MRC at any time may review the Contractors Membership Criteria documentation. MRC may deny an enrollment if it determines that the individual does not meet the Brain Injury Community Center Membership Criteria.A Member may be disenrolled if the Contractor, at any time, determines that a Member poses a significant and current threat to the general safety of the Community program. Prior to disenrolling the member, the Contractor shall consult with MRC as to appropriate actions to be taken, including the process that would be used for reassessing safety.3.3Service OrientationA Member and, when applicable, his/her legal representative, must be provided with a program orientation as soon as it is practical on or after the Members enrollment date. In addition, the Contractor must ensure that Members, legal representatives, and other individuals are provided with written information about the following during orientation and/or upon request:Description of the Community CenterHours of operationConfidentiality policyNondiscrimination provisionsRights and responsibilities of Members including how members can take on the role of staff assistantRight to ask for accommodations, including but not limited to asking for interpretersThe process regarding how an Assessment and Action Plan will be developed with the Member
