Specifications include, but are not limited to:Liaison/Case Management Services for Homeless Singles: Social Services to the Homeless (SSH) Dollars $63,937.00 - July 1, 2016 through June 30, 2017; $63,937.00 July 1, 2017 through June 30, 2018 (with an option for a contract extension for an additional fiscal year (July 1, 2018 through June 30, 2019) Total Contract Amount - $127,874.00 (for two fiscal years) Introduction This RFP seeks to fund a Liaison/Case Manager position facilitating client contact, referral and coordination between the Coordinated Entry and Assessment Services (CEAS) Center and the Rescue Mission of Trenton (MISSION) emergency services program. Key Responsibilities of this position are: Participating in CEAS training, meetings, and case conferencing as requested Screening clients in the emergency services program by utilizing the CEAS screening form, based upon the list of individuals identified as priority cases by the CEAS Center Reviewing the completed HMIS intake for CEAS-identified priority clients with the individual and providing updates as necessary Providing referrals and linkages to providers of benefits, healthcare and other supportive services based on identified needs Coordinating Emergency Services outside service provider schedule and client contact Providing Liaison/Case Management Services per The MISSION and CEAS procedures, including the facilitation of scheduling of CEAS center appointments with clients. Completing case management records in HMIS. Ensuring that service levels are being met and recording service data on the roster Working with the CEAS Team to manage data and monitoring client outcomes 2.) Emergency Shelter Services for Singles: Homeless Assistance (HA) Social Services Block Grant (SSBG) Funding Award Amount: $165,494.00 July 1, 2016 through June 30, 2017 and $165,494.00 July 1, 2017 through June 30, 2018 (with an option for a contract extension for an additional fiscal year (July 1, 2108 through June 30, 2019) Total Contract Amount: $330,988.00 (for two fiscal years)
