Specifications include, but are not limited to:Undertake activities that identify at-risk, eligible households such as housing and-3- health fairs, door to door canvasing, neighborhood events, mailings, and targeted advertising. Hennepin County is not prescriptive as to methods and means of identifying target households provided the activity is legal and is not in conflict with the grants policies, the Grant NOFA, OMB Circular A-122 and HUD Title X. B.Perform in-home lead education visits. 1.Complete visual assessment of the dwelling unit and common areas that documents the building condition, window type, paint condition, and other possible lead and health hazards. (The visual assessment form will be approved by Community Works before work begins and will included other healthy homes hazards other than lead.) 2.Collect 3 lead dust wipe samples from representative surfaces in the dwelling. 3.Take digital photos of example windows and other presumed hazards, and one photo of the exterior front elevation of the house. 4.Provide lead safety education to the occupants based on current best practices. 5.Provide an overview of Hennepin Countys lead hazard reduction grant program. 6.Assist homeowners, rental property owners and tenants with the Hennepin County grant application package including application forms, income documentation and blood lead testing.
