Chafee Independent Living Act of 1999) funding when their foster care case was open (foster care case may still be open), and2)Have been in an eligible foster care or non-secure delinquency placement supervised by the Department of Health & Human Services at age 14 or older. Eligible placements include licensed foster family homes, relative provider homes, group homes, emergency shelters, child care institutions for less than 25 children, independent living, and semi-independent living placements.b.Determination of EligibilityEligibility shall be determined by the Education and Youth Services Unit at DHHS Central Office.1.2.ReservedUse the referral process below or enter an appropriate referral process.2.CONTRACTOR RESPONSIBILITIES2.1.Reserved2.2.CredentialsThe Contractor shall assure that appropriately credentialed or trained staff under its control, including Contractor employees and/or subcontractors, shall perform functions under this Agreement.The Independent Living Skills Coach shall possess a masters degree in one of the following human services areas: human services, social work, sociology, psychology, family ecology, consumer/community services, family studies, family and/or child development, guidance/school counseling, counseling psychology, or criminal justice; be clinically and trauma trained; and have experience in crisis response and conflict resolution.All staff providing services shall have strong interpersonal, organizational, verbal and written communication skills as well as knowledge or understanding of the specific needs that foster youth encounter when entering college.
