Specifications include, but are not limited to:The Arizona Department of Health Services (ADHS), Bureau of Tobacco and Chronic Disease (BTCD), Diabetes Prevention and Control Program (DPCP), intends to grant funding to nonprofit organization(s) in Yuma County, Arizonawhich has established border health partnerships with various health systems and/or community based-organizations, Ventanillas de Salud (Program designed in identifying health services for families in the United States and Mexico), that can provide culturally-competent diabetes education, promote healthy behaviors (nutrition and physical activity), and have expertise in the development of culturally-grounded intervention programs specifically designed to prevent type 2 diabetes among high-risk women and children from vulnerable communities 1.2 The DPCP has concentrated its efforts to reduce the incidence and prevalence of diabetes and the debilitating complications associated with this disease. Obesity and type 2 diabetes disproportionately impact families and communities, and in Yuma County, diabetes is one of the leading causes of early disability along the US/Mexico border. In the US/Mexico border area there is a higher prevalence rate of type 2 diabetes at 15.7% compared with 5.2% in the United States. Unique to the US/Mexico border, community health workers or promotores de salud are trusted members of the community that have been successful in providing culturally and linguistically appropriate diabetes education in diverse settings. For many years, the DPCP has been a strong advocate for the promotora health model and has worked closely with stakeholders and partners, such as the American Diabetes Association, American Heart Association, the National Association of Chronic Disease Directors, university/academic institutions, various hospitals throughout the state; and Federally Qualified Health Centers, to utilize community health workers to accomplish the mission of reducing the burden of diabetes on individuals, families and communities in Arizona
