Specifications include, but are not limited to:IntroductionThe purpose of this agreement is to create a structured, long-lasting and compatible relationship between a mentor and mentored youth that focuses on the needs of the youth and encourages the youth to develop to their fullest potential. Through this partnership, the youth will develop a vision for the future and begin to take action toward personal goals.Programs will pair volunteer mentors and youth based on information such as common interests, hobbies, backgrounds, availability, preferences, life experiences, temperament.1.PROGRAM REQUIREMENTS1.1.Client Eligibility Criteriaa.Definition of Eligibility normally by funding source or program standardsYouth eligible for this contract are youth ages 14 to 21 and currently in foster care as result of an abuse or neglect court action and in an eligible placement, as well as youth aged 18 to 21 who have a closed foster care case and were eligible for YIT funding while the case was open.b.Determination of EligibilityThe Contractor shall accept referrals for eligible youth from the local DHHS offices. Self-referrals may be made, but the local DHHS office must determine eligibility prior to service delivery. DHHS shall determine eligibility based on the YIT Eligibility Checklist from MiSACWIS or the DHHS 722-O or DHHS 722-C and submit to Contractor. DHHS shall provide the most recent Child Assessment of Needs and Strengths (CANS) to the Contractor. Services that should otherwise be provided by the assigned childrens services case worker shall not be performed under this Agreement.
