Specifications include, but are not limited to:The purpose of the EBP program is to provide services designed to enhanceolder adult health, memory, physical activity, and support family caregivers: Improve health and promote healthy life styles; Reduce the impact that physical disorders have on the quality of life ofolder adults; Promote and engage older adults in exercise and other health relatedactivities; and Address issues of incorrect medication and adverse drug reactions.The Contractor is required to tailor an evidence-based service plan to theneeds of the participants representative of each of the respective AgingService Areas.b. The successful proposer will provide EBP programs from the LADOAapproved list (below). The list may be updated as new programs becomeavailable. The successful proposer must provide a minimum of oneevidence-based program from each of the three categories listed below:Chronic Disease Self-Management/Healthier Living: Healthier Living English version Tomando Control de su Salud Spanish version Physical Activity programs: A Matter of Balance (English and Spanish) Active Start (English and Spanish) Aquatic Exercise Association (AEA) ExerciseProgramSM (English and Spanish) AEA) Walk With Ease ProgramSMCaregiver Support or Memory Enhancement: Powerful Tools for Caregivers Savvy CaregiverTM UCLA Memory Trainingc.
