Specifications include, but are not limited to: The Contractor shall provide all labor and resources for the purpose of providing scheduled dermatology services in accordance with the Department of Health licensing standards, the Commonwealth of Virginia DBHDS licensing standard, and the highest professional ethics and standards.1.0Contractor shall be licensed by the Commonwealth of Virginia to practice medicine andbe a Board Certified Dermatologist. Contractor shall further be credentialed/approved by Medicare/Medicaid as a participating provider. Contractor shall maintain and keep current all licenses, certifications, and /or registrations during the period of the contract and any subsequent renewals.2.0Contractor shall conduct dermatology clinics at the Virginia Center for Behavioral Rehabilitation at a mutually agreed upon schedule. Anticipated needs are approximately one (1) clinic every one to two months of four (4) hours duration. Frequency and duration of clinics are subject to change as needs dictate and in accordance with the Department of Health licensing standards.3.0Contractor shall provide prompt patient examination and treatment of dermatological conditions; timeliness shall be determined by mutual agreement of the Agency and the Contractor. Such services may be rendered at the Contractors office or emergency room or at the Agencys facility, at the option of VCBR Medical Director. Timely follow-up care shall be provided.4.0 The Contractor shall provide timely elective, dermatological surgery at a Joint Commission accredited hospital when such surgery is mutually advised by Agencys Medical Director and the Contractor.5.0The Contractor shall provide a written report of outpatient services on VCBR FormMS-25, Request/Report.6.0The Contractor shall provide a written report of inpatient services on Form MS-25, Request/Report. In addition, the Contractor shall assure that copies of operative reports, discharge summaries and other appropriate records are provided to the Agency as requested.7.0The Contractor shall provide Agency telephone consultations and reports to Agencys physicians as requested. Such communication shall be direct, not relayed through office staff or other third parties. The Contractor shall conduct all consultation services in accordance with existing patient confidentially guidelines and regulations, including the publication of statistics or studies.
