Specifications include, but are not limited to: The City of Norwich (City) currently provides health benefits for employees, retirees, COBRA participants, and their dependents as broken out in the Enrollment History below. The City is issuing this Request for Proposals to identify a vendor to assist with verifying dependent eligibility in accordance with the provisions of the Citys plan. Eligible active employee status classifications include full-time, part-time, sick leave, FMLA, military leave, leave of absence, and suspension. Planning Activities1. On-site planning session following award of contract where City personnel will receive a thorough description of the process with understanding of areas where their limited involvement may be required.2. All documents will be customized and presented to the City for approval prior to release. Forms will solicit collection of dependent social security numbers.3. Initial mailing (Notification) notification mailing to announce the verification process.
