It is the intent of the Bureau of Motor Vehicles (BMV) to contract with a vendor that conducts quality customer satisfaction surveys for the BMV. 1.4 SUMMARY SCOPE OF WORK The scope of the services requested in this RFP consists of three primary categories of work: 1) Development of Telephone Survey Instrument, 2) Conducting Telephone Surveys, and 3) Statistical Analysis. The requirements for each category are specified herein. I. Development of Telephone Survey Instrument. A. The Contractor will develop a quarterly telephone survey instrument with no more than fifteen (15) questions. Contractor will work in conjunction with BMV staff to develop questions and will provide consulting and recommendations regarding development of the survey instrument. This instrument will be updated quarterly in order to: 1. Measure customer satisfaction with those who have completed any type of transaction within three (3) months prior to the survey interview
