Specifications include, but are not limited to:The health care provider (contractor) will be responsible for providing all services necessary to perform comprehensive physicals on members of the Countys Fire & EMS Department in accordance with the testing requirements listed herein, provide associated administrative and consultative services, and maintain accurate and detailed records and documentation with which Chesterfield Fire and EMS can measure the success of the health assessment program. Offerors should demonstrate their ability to provide the following: A. Physical Career Firefighter (Pre-Employment) and Volunteer Firefighter (Pre-Entry) The comprehensive physicals administered on Fire & EMS candidates for pre-employment and pre- entry requirements, as determined by the County in conjunction with the Medical Director for Fire & EMS, are administered in two phases and should consist of the following test and services: Phase I Review candidates medical release form and OSHA medical evaluation questionnaire for respirator clearance presented by member at time of examination. Review in-depth patient history including candidates proof of Tdap, MMR, polio, and chickenPage 4 of 23 pox vaccinations or exposure. Also review Hep-B Series, if applicable. Perform hands-on physical examination for evaluation of respiratory, cardiovascular, musculoskeletal, ENT genitourinary and lymphatic systems to critically assess for chemical/physical agent and carcinogenic exposures. Review of vital signs such as height, weight, blood pressure, pulse, exercise pulse, vision and color vision. B. Perform, at a minimum, the following tests: Cardiac screening based on the following: For candidates with 2 or less risk factors and are 34 years of age or younger, perform Level 5 Bruce stress test including EKG with interpretation, interpreted by a cardiologist. For candidates with more than 2 risk factors or are 35 years of age or older, and have not received calcium scoring testing, perform calcium scoring with interpretation, interpreted by a cardiologist. If candidate has had normal previous calcium scoring testing, perform Level 5 Bruce stress test with interpretation, interpreted by a cardiologist. Chest x-ray (PA & LAT) with interpretation by a board certified radiologist. Pulmonary function testing with interpretation and respirator clearance. Assess for possible sub-clinical asthma or other obstructive lung disease. Audiometric testing with interpretation. Executive I Profile blood test. Hepatitis C. HIV screening. Heavy metals testing. Urinalysis (dipstick) to screen for blood sugar, and/or protein in urine. Hemoccult slides to test for colon cancer. Quantiferon blood test(QF-TB). Perform a physical agility/mobility screen specific to fire and EMS personnel.
