Specifications include, but are not limited to:SCOPE OF WORK The Collaborative that is awarded a contract will: Hire or identify 4.0 FTE staff to perform the service components of this RFP: outreach, screening, casefinding,and referrals; and developing community-specific strategies to support families with youngchildren, including, but not limited to: facilitated groups and/or classes, case management, research,advocacy, training for providers. These staffs are referred to here as Community Health Workers(CHWs). CHWs should share culture, language and life experiences of targeted population. In addition,the services of 1.0 FTE Supervisor are required to support staff and program operations and goals.Proposers are also strongly encouraged to budget additional administrative FTE to support data entryand monitoring. Assure that CHWs and Supervisor participate in necessary training: Medicaid, Screening, Supervisor,400004762 Community Health Worker Services for Strategies for Immigrant/Refugee Family Health 13 of 41CHW and/or doula certification training Assure that CHWs receive adequate administrative and reflective supervision. Participate in monthly steering committee meetings with supervisors, CHWs, and MCHD staff Develop a project plan in collaboration with steering committee members, and a work plan for eachCHW based on the specific targeted strategies for the communities they are working with. Thesestrategies may include, but are not limited to: facilitated groups and/or classes, case management,research, advocacy, training for providers. Submit completed screens, tracking forms of CHW activities and services, and documented supervisionforms to MCHD in a timely fashion. Report on progress made in implementing strategies, review and revision, as necessary, of the workplan
