Specifications include, but are not limited to: General RequirementsProvide direct colorectal cancer screening services to a minimum of 175 individuals, including casemanagement, education, outreach, and follow up (including diagnostic and treatment services asfunding allows) for Prince Georges County residents and otherA. Maryland residents as pre-approved. Clients can be symptomatic or asymptomatic at thetime of screening. Individuals to be screened and case managed include those meeting thefollowing eligibility criteria:a. 50-74 years of age, at average risk for colorectal cancer; orb. 18 to 49 years of age, at increased risk for colorectal cancer: Has had any of the following: colorectal cancer in the past, an adenomatouspolyp, inflammatory bowel disease (ulcerative colitis or Crohns colitis), orcancer of the ovary or endometrium Has a mother, father, brother, sister, or child who had colorectal cancer or anadenomatous polyp Has a family history of genetic forms of colorectal cancer or polyps; orc. Over 74 years of age with provider recommendation taking into accountcomorbidities, longevity, and past colorectal screening; andd. Low-income (must not exceed 250% of Federal poverty guideline as defined by theU.S. Internal Revenue Service); ande. Uninsured or underinsured (for screening
