Specifications include, but are not limited to:FOR ALL MAINTENANCE 1) A logbook will be prepared by the Contractor and kept on-site for each piece of equipment to be maintained by the technician. The log shall contain the date each preventative maintenance (PM) and/or repair service was performed. In addition, the log shall contain a description of all repairs. This must include date of service, technicians name, make, brand, model number, name plate data, manufacturers maintenance literature, parts breakdown, and superheat charging tables. Superheat charging tables to be provided by the Contractor when contract begins. This logbook is the property of the CHP and shall not be removed from the premises. 2) Scheduled PM shall be at regular intervals 90 days apart. 3) Contractor must call the facility and make an appointment to perform the PM at least three (3) days in advance. 4) The specifications included in this contract will serve as a checklist for the maintenance services performed. Contractor will be required to bring the checklist each time the PM is performed. 5) All items on the checklist will be checked off as performed. Upon completion, Contractor and the on-site facility designee must sign the checklist and file in the appropriate log. 6) Additionally, an extra copy shall be provided to the CHP Facilities Coordinator and shall accompany the invoice prepared by the Contractor for the PM. 7) The rate for PM shall include filters, fan belts, water treatment chemicals, compressor oil, cleaning rags, oil, grease, and refrigerant/Freon at no additional charge to the state. Costs for the recapture of refrigerant as required by law must also be included. 8) If any parts are identified during the PM that requires replacement, prior approval must be given by the CHP Facilities Coordinator. Additionally, a service report detailing the repairs provided must be signed and dated by both the technician and the site designee. A copy of this report must be filed in the appropriate logbook at the facility and accompany the invoice prepared by Contractor
