Whenever herbicide is utilized within the landscaping, the contractor shall fill out and submit a work order report sheet to SAM Department Representative. The report sheet will be provided by the SAM Department Representative in advance. 11. Any appropriate aggregate from the landscaped median found in the roadway shall be removed from the roadway and reincorporated into the landscaped median. 12. Aggregate shall be kept in a manicured fashion any disturbance shall be raked into place weekly. No weed barrier material should be visible. 13. Any areas that require replenishing of aggregate will be brought to the attention of the City inspector. SAM will provide the aggregate material for the contractor to install. 14. Contractor shall have visible company information on vehicles to include name, address and any license information. 15. Traffic Control in Compliance with Municipal Code (section 12.30) and the Texas Manual of Uniform Traffic Control Devices (TMUTCD). State ROW must conform to requirements outlined in the TMUTCD, specifically the use of an attenuator on State ROW. A copy of the approved Traffic Control Plan must be kept at the working location at all times and available to the City inspector if requested. 16. The Contractor shall inspect all trees and shrubs within landscaped areas weekly, and shall notify the SAM Department Representative in writing of problems with insects, diseases, or animals as such problems arise. Treatment of the plants or planted areas shall be accomplished as directed by the SAM Department Representative. Any infestations shall be immediately reported to the SAM Representative
