Specifications

Specifications include, but are not limited to:Many persons in the City of San Diego who have SMI and/or serious substance abuse diagnosis are homelessand have not successfully accessed resources to support recovery. This project will provide 100 Medi-Caleligible persons with mental health and/or substance abuse services through both direct service and linkages torelevant services, and will support them to successfully access subsidized housing resources provided throughthe San Diego Housing Commission (SDHC) within the city of San Diego. Clients must meet the HUDdefinition for homelessness and live within the geographic limits of the City of San Diego. Potential clientsmust meet the eligibility requirements for housing subsidies through SDHC.The Housing First approach of this program will ensure clients are provided access to good quality housing.The program will provide relevant treatment and support services to help ensure clients have access to theresources they need to promote recovery. Services will be provided at both the program site and in thecommunity, and two full-time Housing Coordinators shall be included in the program staffing.Assertive Community Treatment (ACT) model services have proven to be a very relevant service model forpersons with SMI due to the intensity and range of available services through this model. It is also expectedthat persons will be linked with relevant community services as indicated to help promote increased selfsufficiency.Alcohol and Other Drug (AOD) services are expected to be provided through a combination of outreach andengagement, case management, recovery services/treatment/support services and linkage to relevantcommunity services (e.g., Regional Recovery Centers, Residential Treatment, and other relevant socialsupports). Funding is provided through a combination of Drug Medi-Cal, Substance Abuse Prevention andTreatment (SAPT), and Realignment funding for persons with AOD diagnosis with expectation for revenuegeneration through billing of all eligible Drug Medi-Cal billable substance abuse services. It is also expectedthat persons will be linked with relevant community services as indicated to help promote increased selfsufficiency.Services will be provided at both the program site and in the community, and at least one full timelicensed mental health clinician shall be included in the program staffing for this component.
